I am trying very hard to drop this topic but since you raise some
specific questions:

I don't think the "invisible hand of the marketplace" as you suggest
can work in last-mile services in most places simply because they are
small N (often N=1) oligopolies all with basically the same policies.

That's a problem which spans net neutrality and many other issues.
